Long-term incapacity for work in Germany may lead to a situation in which a German health insurance fund requires the patient to submit an application for medical rehabilitation to Deutsche Rentenversicherung.
Such a request may be received by a person insured with, among others, AOK, TK, BARMER, DAK-Gesundheit, IKK, BKK or another German health insurance fund, particularly if they have been unable to work for a longer period and are receiving Krankengeld.
A letter from the health insurance fund is not merely an offer to undergo rehabilitation. It usually means that the patient’s continued ability to work has been assessed as being at risk or already reduced, and that rehabilitation may improve their health and increase their chances of returning to professional activity.
The patient usually receives an extensive set of forms in German, including forms G0100 and G0110, as well as a specific deadline for submitting them. The documents should not be postponed, because failure to respond or missing the deadline may result in the suspension of Krankengeld payments.

Why does a German health insurance fund require a rehabilitation application?
If a patient remains unable to work for an extended period, the health insurance fund may commission Medizinischer Dienst to assess their health condition and professional prognosis.
If the assessment shows that the patient’s ability to work is seriously at risk or already reduced, the health insurance fund may require the insured person to submit an application for medical rehabilitation or other benefits aimed at maintaining or restoring their earning capacity.
In such a case, the patient receives a specified deadline for submitting the documentation to Deutsche Rentenversicherung. Information confirming that the application has been submitted should also be provided to the health insurance fund.
The “Reha vor Rente” principle – rehabilitation before pension
The German social insurance system follows the principle:
Reha vor Rente – rehabilitation before pension
This means that Deutsche Rentenversicherung first examines whether the patient’s health condition and ability to work can be improved or restored through medical or occupational rehabilitation.
Only if rehabilitation is unlikely to result in sufficient improvement may the granting of a pension due to partial or full reduction in earning capacity, known as Erwerbsminderungsrente, be considered.
A rehabilitation application is therefore not merely a referral for treatment. The documentation may be important for the further assessment of:
- the patient’s health condition,
- the course of treatment,
- the duration and causes of incapacity for work,
- the burdens associated with the patient’s occupation,
- the possibility of returning to the previous job,
- the need for vocational retraining,
- the justification for continued payment of benefits,
- a possible later pension procedure.
For this reason, the forms should be prepared accurately, consistently and in line with the medical documentation.
How much time does the patient have to submit the application?
A German health insurance fund may set a specific deadline for the patient to submit a rehabilitation application. In practice, this is often a period of 10 weeks from the date the request is delivered.
It is not advisable to wait until the final days. Preparing a complete application may require:
- analysis of the letter from the health insurance fund,
- collection of medical documentation,
- obtaining information from the doctor,
- correct description of the course of the illness,
- presentation of the professional situation,
- selection of the appropriate attachments,
- indication of a preferred rehabilitation facility,
- collection of the patient’s signatures.
Even if the application is sent shortly before the deadline, the health insurance fund may not receive confirmation of its submission in time. This may lead to the temporary suspension of Krankengeld payments until the matter is clarified.
What happens if the documents are not submitted on time?
Failure to submit the application within the deadline may have serious financial and procedural consequences.
These may include:
- suspension of Krankengeld payments,
- loss of entitlement to the benefit for a specific period,
- inability to obtain retroactive payment of the benefit,
- delay in the rehabilitation procedure,
- the need to provide additional explanations,
- problems resulting from a lack of cooperation with the health insurance fund and DRV,
- refusal or withdrawal of a benefit if the required documents are not provided.
Simply informing the health insurance fund that the patient intends to submit an application may be insufficient. It is usually necessary to actually submit the complete documentation to Deutsche Rentenversicherung and provide confirmation to the insurer.
Form G0100 – the main rehabilitation application
G0100 is the basic form used when submitting an application for rehabilitation benefits to Deutsche Rentenversicherung.
The document includes, among other things:
- personal and address details,
- the insurance number with Deutsche Rentenversicherung,
- information on whether the application was requested by the health insurance fund, Agentur für Arbeit or Jobcenter,
- selection of the type of rehabilitation,
- information on employment and occupation,
- information on the current professional situation,
- details of the health insurance fund,
- details of treating doctors,
- periods of contribution payments in Germany and abroad,
- information on previous rehabilitation,
- information on benefits and pensions,
- information on accidents or occupational diseases,
- details of an authorised representative,
- consent to obtain medical documentation,
- preferences regarding the rehabilitation facility.
Form G0100 forms the basis of the entire procedure. Errors, omissions or inconsistencies may extend the process and result in the need to provide additional explanations.
Form G0110 – information on health and work
G0110 is an attachment to the application for medical rehabilitation. It is used to present the patient’s health and professional situation.
The form includes, among other things:
- periods of incapacity for work,
- main illnesses and symptoms,
- coexisting medical conditions,
- recognised degree of disability,
- prognosis regarding future work,
- the occupation performed,
- physical and psychological burdens,
- standing, sitting or working in forced positions,
- lifting and carrying heavy objects,
- shift work or night work,
- work under time pressure,
- exposure to noise, dust, temperature or chemicals,
- difficulties in everyday functioning,
- expectations regarding rehabilitation.
Form G0110 should be completed in accordance with the actual state of health and the nature of the work performed. The answers should not be random or too general.
What additional attachments may be required?
The scope of the documentation depends on the patient’s individual situation. In addition to forms G0100 and G0110, other documents may also be required.
These most often include:
- G0105 – an attachment concerning cross-border situations,
- a medical report prepared by a doctor,
- hospital treatment documentation,
- test results,
- hospital discharge summaries,
- certificates from specialist doctors,
- documents concerning previous rehabilitation,
- the letter from the German health insurance fund,
- confirmation that the application has been submitted,
- a power of attorney,
- employment documents,
- documents confirming periods of insurance in Poland or another country.
Not every patient has to submit all attachments. It is necessary to determine which forms are required in the individual case.
Can the patient choose their own rehabilitation clinic?
The patient may indicate a preferred rehabilitation facility. Several facilities may be listed in the form in the order corresponding to the patient’s preferences.
When choosing a clinic, it is worth taking into account:
- the main medical condition,
- the scope of therapy,
- the facility’s experience,
- availability of appointments,
- distance from the place of residence,
- the possibility of communicating in Polish,
- accessibility for persons with disabilities,
- the possibility of treating coexisting conditions.
If the patient does not indicate a preferred facility, Deutsche Rentenversicherung may propose an appropriate rehabilitation centre.
Can rehabilitation be refused?
If the health insurance fund has formally required the patient to submit an application, refusal of rehabilitation should not be decided upon without first analysing the situation.
Withdrawal from the procedure for family, organisational or professional reasons may not be considered sufficient and may affect the continued payment of Krankengeld.
Later actions such as:
- withdrawing the application,
- postponing the rehabilitation start date,
- changing the facility,
- refusing to begin treatment,
may also affect continued entitlement to benefits. Before making such a decision, it is advisable to contact the health insurance fund and Deutsche Rentenversicherung.
